Background: Theobjectiveistocomparetheamountofpainingeneralanesthesiaonlywiththatofgeneralanesthesiaplusinterscaleneblock. Weanalyzedtocomparethefactoraffectingthepost-oppainarousalwiththehematologicalfactorsthatarerelatedtothepostop pain Methods: 62consecutivepatientswhowerediagnosedthe2~4cmsizedcufftear,thisstudywasdesignedbyprospective,randomized design.Thepatientswererandomlydividedintothegroupofgeneralanesthesiaonly(GAgroup)andinterscaleneblock(ISB group).ISBblockwasgiventheinjectionof2%lidocaine15mland2%levobupivacane15ml. Thesurgerywasperformedusingarthroscopicsuturebridgetechnique. ThepainVASwasmeasuredinallgroupsonthePREOPday,OPdayanduntilPOD3.Theintensityofpainwasmeasured continouslymeasuredevery6hoursuntilPOD3usingbyNRS-11(elevenpointnumericratingscale). TheNRSpointafterpostoperativehoursof12wasconsideredasaprimaryefficacyevaluationvariable Bloodsamplingwasconductedtoevaluatethefluctuationofthestresshormoneatevery8AMontheaPREOPday,OPdayand POD1,2,3 Insulin,DHEA-S,Fibrinogenwhichthoughttobethepainrelatedfactorswererecorded. Results: VASwhichmeasuredatthePOD1,alsorevealedremarkablylesspainintensityontheISBgroup(p<0.01).Thereisnosignificant differencebetweentwogroupsatthePOD2and3(p>0.05).BecauseofthestatisticaldifferenceatthePOD1,therefore,thepain intensityofthetwogroupwassubdividedintotimeinterval,then,theresultsrevealedthatVASintheISBgroupisremarkably lowerthantheGAgrouponly(p<0.05). ThereisnodifferenceintheInsulin,DHEA-SandFibrinogenamongtwogroupsfromPREOPdaytoPOD3(p>0.05) Thevalueofthethreehormoneshasalsonostatisticaldifferencesaccordingtothesexandinterscaleneblockapplication. Conclusions: Afterarthroscopicrotatorcuffrepair,painVASwassignificantlylowerontheGAwithISBgroupthangeneralanesthesiaonly group,butInsulin,DHES-A,fibrinogenaspainrelatedhormoneswerenotfoundanypostoperativelystatisticalsignificance.
