[Objective] To evaluate the therapeutic effect of different methods for cervical spinal cord injury without fracture and dislocation in old and middle-aged patients in order to provide feasible therapic strategy to clinic.[Methods] A retrospective study was designed.Data of patients meeting the demands were collected: 50 years and above,admission between January,2004 and December,2009, meeting the diagnosis of cervical spinal cord injury without fracture and dislocation ,follow -up at least 6 months.Mechnism of injury,neurological function impairment,characteristics of imaging,treatment methods and therapeutic effect were analyzed.[Results] All cases were followed up for 10.8 uticmonths (6 ~ 18 months).According to Frankel classification for spinal cord injury,there were obvious improvements for 12 cases from Grade A to B or C, 16 cases from Grade B to C or D, 24 cases from Grade C to D or E, 21 cases from Grade D to E.The JOA (Janpanese Orthaepnic Association) scores amplification of prior and post treatment differed between operation group and non-operation group significantly (P < 0.05) ,and the same was between the one posteriorapproach group and the other posterior-approach group (P < 0.05).There was no statistical difference among anterior-approach group, posterior-approach group and anteriorposterior combined approach group (P > 0.05).[Conclusions] It might obtain the satisfactory therapeutic effect to take various "individualized methods" to treat cervical spinal cord injury without fracture and dislocation in old and middle-aged patients complicated with cervical degeneration at different levels,The key is to take comprehensive evaluation for preoperative informations of patients, to choose reseasonable surgery approach and occasion for better therapeutic effect
