Various nasal characteristics must be considered when planning and executing ethnic rhinoplasty. The Asian nose typically has a low, flat nasal dorsum and insufficient nasal tip projection with a lack of tip definition because of weak cartilages and thicker sebaceous skin.Although autogenous grafts of cartilage continues to be the preferred source of grafts in white rhinoplasty, alloplastic materials such as silicone and expanded polytetrafluoroethylene (ePTFE) are most commonly used as powerful tool in dorsum augmentation for improving cosmesis in Asia.ePTFE has won increasing favour over the last few years in Asia, which is believed to be the better and easier choice resulting in naturally cosmetic result. However, insufficient nasal tip projection after augmentation rhinoplasty with ePTFE is still common problem, being afraid of unacceptably high extrusion rates especially when they are used for structural support of tip.The study is aimed at developing some surgical techniques and introducing our experience using ePTFE in nasal augmentation of the tip and dorsum. Also, autogenous grafts of cartilage, such as conchal cartilage or/and nasal septal cartilage graft were used in another series of Asian patients for nasal augmentation of the tip.We will compare the biocompatibility, ease of using and shaping, extrusion or resorption, pliability, rate of infection with ePTFE and autogenous grafts of cartilage, not only for height of dorsum but also projection and lengthening of nasal tip.
