Objective:To evaluate the surgical procedure of unilateral transforminal discectomy,bone grating,cage(Telamon) insertion and Dylanok pedicle screw fixation using X-tube operation system for the treatment of lumbar disc herniation combined with segmental instability and spondylolithesis with pars defect.Methods:From 2004 to 2006,42 patients including 17 male and 25 female were treated in our department.The age range from 22 to 77 (mean:51.6).Etiologies including L4,5 extraforaminal disc herniation (9 cases),L5S 1 extraforaminal disc herniation (5cases),L4,5 disc herniation combined with degenerative spondylolithesis (6casesL5S1 disc herniation combined with degenerative spondylolithesis (7 case),L4 pars defected combined with spondylolithesis(Ⅰ °) (5 cases),L5 pars defect combined with spondylolithesis(Ⅰ °) (6cases),recurrent lumbar disc hemiation at L4,5 level(2 cases),and recurrent lumbar disc herniation at L5Sllevel (2 cases).
