女,42岁,1979年3月中旬因右上第一大臼齿疼痛、过敏,行脱敏治疗时局部粘膜灼伤,形成约0.5×1cm溃疡,敷四环素甘油后痊愈。同年4月7日因右上第一大臼齿疼痛,拍片根端有破坏,在局麻下拔除。手术顺利,拔牙三周后患者觉口周有紧缩感,张口较困难,口周发麻,说话吐字不清,面部、四肢有抽动,以破伤风之诊断于5月8日急诊收入外科。入院后症状渐重,除上述症状外,自觉颈项部较硬,有轻度抽动感。查体:发育营养佳,神志清,自动体位,脉搏70次/分,呼吸20次/分,血压110/70毫米汞柱,除张
