目的:研究结肠癌急性肠梗阻患者术后感染的病原菌分布及耐药性,为临床合理使用抗菌药物提供依据。方法选取2011年1月-2013年12月结肠癌急性肠梗阻术后发生切口感染248例患者,采集切口感染样本进行病原菌培养,并进行鉴定及药敏分析。结果4925例患者有248例术后发生切口感染,感染率占5.03%,248例患者共检出病原菌234株,革兰阴性菌检出125株,占53.42%,革兰阳性菌检出98株,占41.88%,白色假丝酵母菌11株,占4.70%;大肠埃希菌、肺炎克雷伯菌和铜绿假单胞菌对亚胺培南、阿米卡星以及头孢哌酮的敏感率较高,分别为95.20%、92.80%以及88.00,对磺胺甲噁唑/甲氧苄啶及头孢拉唑的敏感率较低,分别为10.40%和9.60%;金黄色葡萄球菌、肺炎链球菌及表皮葡萄球菌对万古霉素、替考拉宁的敏感率极高,分别为97.96%及92.86%,对青霉素、磺胺甲噁唑/甲氧苄啶的敏感率较低,分别为8.16%、7.14%。结论治疗结肠癌并发急性肠梗阻术后感染患者,应根据药物敏感试验结果合理应用有效的抗菌药物。“,”OBJECTIVE To study distribution and drug resistance of pathogens causing infections in colon cancer pa‐tients after received acute intestinal obstruction operation so as to provide the basis for clinical rational use of anti‐microbial drugs .METHODS From Jan .2011 to Dec .2013 ,totally 248 cases of colon cancer patients appearing in‐cision infections after received acute intestinal obstruction operation were collected .The infected incision samples were collected to cultivate pathogens and make identification and drug sensitivity analysis .RESULTS Totally 248 cases had incision infections in the 4925 cases of patients and the infection rate was 5 .03% .Totally 234 strains of pathogens were detected ,including 125 strains of gram‐negative bacteria ,accounting for 53 .42% ,98 strains of gram‐positive bacteria ,accounting for 41 .88% ,and 11 strains of Candida albicans ,accounting for 4 .70% .Esch‐erichia coli ,K lebsiella pneumoniae and Pseudomonas aeruginosa were highly sensitive to imipenem ,amikacin and cefoperazone ,reaching 95 .20% ,92 .80% and 88 .00% respectively ;while less sensitive to sulfamethoxazole/trim‐ethoprim and cephalosporin ,respectively ,10 .40% and 9 .60% .Staphylococcus aureus ,Streptococcus pneumoni‐ae ,and Staphylococcus epidermidis were highly sensitive to vancomycin ,and teicoplanin ,respectively ,97 .96%and 92 .86% . They were resistant to sulfamethoxazole/trimethoprim , respectively , 8 .16% and 7 .14% . CONCLUSION Colon cancer patients with incision infections after acute intestinal obstruction operations should reasonably choose effective antibiotics according to the results of drug sensitivity tests .
