目的探讨组织多普勒成像(TDI)测定左心室Tei指数及E/(Ea×Sa)评价早期无明显临床心血管症状的系统性红斑狼疮(SLE)活动期患者的左心室功能价值。方法30例SLE活动期患者(SLE组)和27例正常人(对照组)行常规超声心动图及TDI检查,测量二尖瓣口血流频谱E峰、A峰;应用定量组织速度成像(QTVI)分别测量受检者二尖瓣环水平6个室壁节段收缩期S峰(Sa)、舒张期E峰(Ea)、A峰(Aa)峰值速度,计算E/A、Ea/Aa、E/Ea、E/(Ea×Sa)值;同时测量各节段等容舒张期时间(IRT)、等容收缩期时间(ICT)、射血时间(ET),计算Tei指数;以上参数均取6个室壁节段平均值进行两组对比分析。结果与对照组比较,SLE组Aa、Tei指数、IRT、E/Ea、E/(Ea×Sa)均明显升高(P<0.01或P<0.05),Ea、Ea/Aa、ET值均明显减小(P<0.01)。E/(Ea×Sa)及E/Ea与Tei指数呈正相关(r=0.523,P<0.01;r=0.323,P<0.05),E/(Ea×Sa)及E/Ea与Ea呈负相关(r=-0.605、r=-0.726,P均<0.01)。结论Tei指数及E/(Ea×Sa)有可能作为评估SLE活动期患者左心室功能减低的早期指标。
Objective To investigate the value of left ventricular (LV) function in patients with systemic lupus erythematosus (SLE) during the early stage of tissue Doppler imaging (TDI) measurement of left ventricular Tei index and E / (Ea × Sa) Methods Echocardiography and TDI were performed in 30 active SLE patients and 27 normal controls (control group).
