目的 研究视网膜脱离复位手术硅油填充后各种注视状态下的眼球屈光变化。 方法 采用Gullstrand模型眼参数进行以下分析 :①非调节状态下正常眼球与硅油填充后眼球的屈光状态 ;②调节状态下正常眼球与硅油填充后眼球的屈光状态 ;③无晶状体眼玻璃体腔填充后的屈光状态 ;④正常眼球与填充硅油后视网膜像放大率变化 ;⑤硅油填充后的白内障人工晶状体植入术人工晶状体度数的变化。 结果 ①与正常眼球屈光相比 ,硅油填充后 ,眼球趋向远视 ,总屈光度变化为 + 9 19D ;无晶状体眼硅油填充后更趋远视化 ;②硅油填充后如将眼屈光矫正至正视 ,调节幅度减少 1 42D ;③硅油填充后常规人工晶状体度数计算公式不再适用 ,可以通过计算推知合适的晶状体度数。 结论 视网膜脱离复位术并硅油填充后眼球屈光发生了高度远视趋向变化 ,调节幅度明显下降 ,常规人工晶状体的有效度数计算在硅油填充后将不再适用。
Objective To study the changes of refraction of eyeball under various gaze after silicone oil refill surgery. Methods The Gullstrand model was used to analyze the following parameters: (1) the refraction of the eyeball after normal eyeballs and silicone oil were filled in the non-adjusted state; (2) the refractive state of the eyeball after the normal eyeballs and silicone oil were filled in the adjusted state; (3) After refractive status; ④ normal eye and filled with silicone oil retina magnification changes; ⑤ silicone oil filled cataract surgery intraocular lens IOL changes. Results ① Compared with normal refractive eyeball, the eyeball tends to hyperopia after filling with silicone oil, the total diopter change is +9 19D; the lensless eye oil is more hypervigit when filled with silicone oil; ② After silicone oil refill, The adjustment range to reduce the 1 42D; ③ silicone oil filled conventional lens intraocular lens formula is no longer applicable, you can deduce the calculation of the appropriate degree of lens. CONCLUSIONS: Retinal detachment and silicone oil refill have a high degree of hyperopic hyperreflexia, and the amplitude of adjustment is obviously decreased. The effective degree of conventional intraocular lens will not be applicable after silicone oil filling.
