目的 探讨超声引导下精准穿刺活检对周围型肺小结节(直径≤2 cm)诊断中的应用价值.方法 回顾性分析我院经CT检查发现周围型肺小结节,且在超声引导下行肺穿刺患者68例,其中高频超声引导39例,低频超声引导29例,按照结节大小及结节与胸膜贴合处直径不同分为两组,分别比较高频超声与低频超声引导下对周围型肺小结节穿刺活检的成功率.结果 结节与胸膜贴合处直径≤1.0 cm时,高频超声引导穿刺成功率为93.8%,低频超声引导穿刺成功率为55.6%,差异有统计学意义(P<0.05).结节长径≤1.5 cm时,高频超声引导穿刺成功率为95.2%,低频超声引导穿刺成功率为64.3%,差异有统计学意义(P<0.05).结论 超声引导下经皮穿刺周围型肺小结节成功率较高,对于结节与胸膜贴合处直径≤1.0 cm的患者,应用高频超声引导肺穿刺更安全且成功率更高,即使在较小的结节(≤1.5 cm),用高频超声引导也能取得较好的效果.“,”Objective To investigate the application value of ultrasound-guided biopsy for small peripheral lung nodules.Methods Sixty-eight patients with small peripheral lung nodules detected by CT scan and underwent ultrasound-guided biopsy were analyzed retrospectively.High frequency ultrasound-guided biopsy was performed for 39 patients and low frequency for the other 29 patients.Two groups were divided according to the size of nodules and the diameter of nodule fitted in the pleura.The success rate of ultrasound-guided biopsy for the small peripheral lung nodules was compared between these two groups.Results In case of the diameter of nodule fitted in the pleura less than 1.0 cm,the success rate of high frequency ultrasound-guiding was 93.8%,and 55.6% for the low frequency.There was a significant difference between these two methods (P<0.05).In case of the size of nodule less than 1.5 cm,the success rate of high frequency ultrasound-guiding was 95.2%,and 64.3% for the low frequency.There was a significant difference between these two methods (P<0.05).Conclusions Ultrasound-guided biopsy for the small peripheral lung nodules has high success rate.For the diameter of nodule fitted in the pleura less than 1.0 cm or the size of nodule less than 1.5 cm,the high frequency ultrasound-guiding may be more safe and effective.
