男,59岁,退休工人。头晕、头痛20余年,胸闷、心慌10余年,上述症状加剧伴呕吐1天,于1988年11月6日入院。患者于1952年冬右侧头部被石块击伤,并出现过一过性昏迷。此后常感头晕,头痛伴失眠,视力逐渐减退和小便失禁。近10年来常感胸闷、心慌。心电图检查示频发室早。起病后曾到省、地级医院求治多次,均未明确诊断,经用慢心律、苯妥英钠等抗心律失常药物治疗及对症处理,效果不佳。入院前1天上述症状加剧,伴恶心,呕吐,吐出物为胃内容物。嗜睡。无发热及肢体抽搐。无高血压、甲亢及风心等病史。
