表“头(身体最上部)”之义时,上古主要用“首”。“头”最早见于《左传》,但使用上处于劣势;从战国中晚期开始,“头”的使用渐趋活跃;经过两汉时代的激烈竞争,“头”以其表义单纯的特点,使用渐多,从而在语法功能、适用对象、搭配组合能力等方面都表现出了明显的优势。结合“以今语、通语释古语”性质的训诂专著《说文解字》以及更加贴近汉代语言实际的中医典籍来看,应该说最晚到汉代末期,“头”已经代替了“首”。
