目的探讨2型糖尿病患者外周血单个核细胞TLR2介导的免疫功能。方法应用流式细胞术检测35例2型糖尿病患者和30名对照人群外周血淋巴细胞、单核细胞、粒细胞TLR2的表达。将两组人群外周血单个核细胞进行体外分离、培养,经TLR2配体HKLM刺激后,检测TLR2的表达及TNF-α的分泌。结果对照组外周血淋巴细胞、单核细胞TLR2表达为(1.75±0.50)%、(28.38±13.92)%低于糖尿病组的(2.12±0.84)%、(35.09±12.69)%(P<0.05)。HKLM刺激后,对照组淋巴细胞、单核细胞TLR2表达为(7.76±2.31)%、(58.74±15.76)%与糖尿病组TLR2表达(4.67±1.54)%、(56.32±16.34)%均有增加(P<0.05);HKLM刺激前后,经配对t检验分析,对照组淋巴细胞、单核细胞TLR2表达的上调高于糖尿病组(P<0.05)。用HKLM刺激对照组、糖尿病组单个核细胞TNF-α的分泌为(83.67±40.87)pg/mL、(40.82±32.52)pg/mL,高于用PBS刺激单个核细胞的(14.32±9.87)pg/mL、(18.36±11.27)pg/mL(P<0.05);对照组TNF-α分泌的增加高于糖尿病组(P<0.05)。结论 2型糖尿病患者外周血单个核细胞TLR2表达升高,但其介导的细胞免疫功能受损。
Objective To investigate the TLR2-mediated immune function of peripheral blood mononuclear cells in type 2 diabetic patients. Methods Flow cytometry was used to detect the expression of TLR2 in peripheral blood lymphocytes, monocytes and granulocytes in 35 patients with type 2 diabetes mellitus and 30 controls. Peripheral blood mononuclear cells were isolated and cultured in vitro. After TLR2 ligand HKLM stimulation, TLR2 expression and TNF-α secretion were measured. Results The expression of TLR2 in peripheral blood lymphocytes and monocytes in the control group was (1.75 ± 0.50)%, (28.38 ± 13.92)% lower than that in the diabetic group (2.12 ± 0.84)%, (35.09 ± 12.69)% (P <0.05) . After HKLM stimulation, the expression of TLR2 in lymphocytes and monocytes in control group was (7.76 ± 2.31)%, (58.74 ± 15.76)%, and that in diabetic group was 4.67 ± 1.54% and (56.32 ± 16.34)%, respectively P <0.05). Before and after HKLM stimulation, the TLR2 expression in lymphocytes and monocytes in the control group was significantly higher than that in the diabetic group (P <0.05). The secretion of TNF-α in the mononuclear cells of diabetic group was (83.67 ± 40.87) pg / mL and (40.82 ± 32.52) pg / mL, respectively, which was higher than that of the mononuclear cells stimulated with HKLM (14.32 ± 9.87) pg (18.36 ± 11.27) pg / mL (P <0.05). The increase of TNF-α in the control group was higher than that in the diabetic group (P <0.05). Conclusion The expression of TLR2 in peripheral blood mononuclear cells of patients with type 2 diabetes mellitus is increased, but the cellular immune function is impaired.
