为探讨Th1 /Th2类细胞因子在慢性乙型肝炎患者肝内及外周血单个核细胞 (PBMC)的表达及其临床意义 ,应用双抗体夹心法和免疫组织化学方法对 63例慢性乙肝患者中Th1 /Th2类细胞因子表达水平进行检测。结果显示 :63例慢性乙型肝炎患者肝组织中IFN γ阳性 46例 ,阳性率为 73.1 % ;IL 4阳性 39例 ,阳性率为61 .9%。阳性染色定位于细胞浆 ,呈棕黄色颗粒。阳性细胞主要分布在汇管区及小叶内坏死区。IFN γ的表达强度与肝内炎症分级呈正相关 ,提示 :IFN γ的表达增强能够促进肝细胞的损伤 ;而IFN γ和IL 4的表达与ALT的水平无相关性。肝组织内IFN γ的表达水平与外周血PBMC培养上清液中的水平是一致的 ,而IL 4缺乏一致性。提示 :①慢性乙型肝炎肝组织内IFN γ表达阳性率高于IL 4;②检测外周血PBMC培养上清液中IFN γ的水平可能更有利于反应慢性乙型肝炎患者的Th细胞应答状态。
To investigate the expression of Th1 / Th2 cytokines in peripheral blood mononuclear cells (PBMCs) and their clinical significance in patients with chronic hepatitis B, the Th1 / Th2 cytokines were detected by double antibody sandwich method and immunohistochemistry in 63 patients with chronic hepatitis B / Th2 cytokine expression levels were detected. The results showed that 46 cases of IFNγ were positive in liver tissue of 63 patients with chronic hepatitis B, the positive rate was 73.1%; the positive rate of IL 4 was 61.9%. Positive staining located in the cytoplasm, brownish-yellow particles. Positive cells are mainly distributed in the portal area and lobular necrosis area. The expression of IFN-γ was positively correlated with the grade of intrahepatic inflammation, which indicated that the enhanced expression of IFN-γ could promote the injury of hepatocytes. However, the expression of IFN-γ and IL-4 had no correlation with the level of ALT. The expression of IFN-γ in liver tissue was consistent with the level in PBMC culture supernatant of peripheral blood, while IL-4 lacked consistency. It is suggested that: ①The positive rate of IFNγ expression in chronic hepatitis B liver tissue is higher than that of IL 4; ②The detection of IFNγ level in PBMC culture supernatant of peripheral blood may be more beneficial to the response of Th cells to chronic hepatitis B patients.
