目的:总结主动脉夹层临床特点,以利于早期诊断,避免误诊。方法:回顾性分析9例主动脉夹层患者临床资料。结果:本组首发症状为胸痛者1例,腰背痛2例,腹痛5例,晕厥1例;血压升高7例、降低1例,呼吸困难、双肺底湿啰音1例,心界增大2例,心脏杂音3例,腹部血管杂音1例,腹部压痛2例,休克1例,双上肢血压不对称1例;8例误诊;死亡6例,好转1例,转院2例。结论:主动脉夹层症状多变、复杂,易误诊,病死率高。
Objective: To summarize the clinical features of aortic dissection in order to facilitate early diagnosis and avoid misdiagnosis. Methods: The clinical data of 9 patients with aortic dissection were retrospectively analyzed. Results: The first symptom of this group were chest pain in 1 case, low back pain in 2 cases, abdominal pain in 5 cases and syncope in 1 case. Elevated blood pressure in 7 cases, decreased in 1 case, dyspnea, double lung wet rales in 1 case, 2 cases increased, 3 cases of heart murmur, 1 case of abdominal vascular murmur, 2 cases of abdominal tenderness, 1 case of shock, and 1 case of asymmetric blood pressure in both upper extremities; 8 cases were misdiagnosed; 6 cases died, 1 case improved and 2 cases transferred to hospital. Conclusion: The symptoms of aortic dissection varied, complex, easily misdiagnosed, high mortality.
