目的 探讨螺旋CT血管造影(SCTA) 检查颅外颈动脉狭窄的准确性以及用SCTA代替数字减影血管造影(DSA)的可能性。方法 本组研究采用NASCET标准对照比较SCTA 与DSA 的颈动脉血管造影结果,并参考部分病例的手术所见,以分析SCTA 检查颅外段颈动脉狭窄的准确性。结果 本组29 例患者的33 个颈动脉狭窄中,SCTA对照DSA的诊断总符合率为87-9 % 。其中严重狭窄组的相符率为100 % 。结论 SCTA不仅对颈动脉狭窄程度的诊断有很高的准确性和直观性,而且能够显示DSA 难以发现的附壁钙化以及血栓。有望取代DSA 在颈动脉狭窄诊断中的地位。
Objective To investigate the accuracy of spiral CT angiography (SCTA) in detecting extracranial carotid artery stenosis and to replace SCTA with digital subtraction angiography (DSA). Methods This study used NASCET standard comparison of SCTA and DSA carotid angiography results, and reference to some cases of surgery seen in order to analyze the accuracy of SCTA examination of extracranial carotid stenosis. Results Among the 33 carotid stenoses in 29 patients in this study, the overall coincidence rate of diagnosis of DSA with SCTA was 87-9%. In which severe stenosis group matched 100%. Conclusion SCTA not only has a high accuracy and intuition for the diagnosis of carotid artery stenosis, but also can display the dilated calcification and thrombus which is difficult to find by DSA. It is expected to replace DSA in the diagnosis of carotid artery stenosis.
