目的研究多巴胺联合酚妥拉明对小儿肺炎合并心力衰竭的疗效及心肌酶的影响。方法选取86例小儿肺炎合并心衰患儿为研究对象,将上述患儿抽签随机分为观察组与对照组,每组43例,两组患儿入院后行镇静、吸氧、抗生素等常规治疗,对照组采取酚妥拉明静脉推注治疗,观察组在对照组基础上联合多巴胺治疗,两组均治疗1周为1个疗程,比较两组治疗1个疗程后临床疗效以及治疗前后乳酸脱氢酶(LDH)、肌酸激酶同工酶(CK-MB)水平变化,比较两组症状缓解时间。结果观察组治疗总有效率为95.35%,与对照组(69.77%)比较显著较高(P<0.05);1个疗程后观察组LDH及CKMB分别为(91.25±12.37)U/L、(30.11±5.27)U/L,与对照组[(121.58±14.03)U/L、(46.31±6.08)U/L]比较显著较低(P<0.05);观察组呼吸困难、烦躁、肺湿啰音、紫绀缓解时间分别为(3.02±1.25)h、(2.74±0.25)h、(91.25±21.05)h、(4.26±0.95)h,与对照组[(6.44±1.48)h、(4.96±1.32)h、(125.85±33.96)h、(9.58±1.25)h]比较显著较低(P<0.05)。结论多巴胺联合酚妥拉明治疗小儿肺炎合并心力衰竭近期疗效显著,可缩短症状缓解时间,恢复心肌酶水平,但有关其远期疗效仍需进一步研究。
Objective To study the effects of dopamine combined with phentolamine on the therapeutic effect and myocardial enzymes in children with pneumonia complicated with heart failure. Methods Totally 86 children with pneumonia complicated with heart failure were enrolled in this study. Conclusion Dopamine combined with phentolamine treatment of children with pneumonia complicated with heart failure recent significant effect, can shorten the time to symptom relief and restore myocardial enzyme levels, but its long-term efficacy needs further study.
