目的探讨不同切口白内障术后角膜散光及泪液分泌变化影响。方法选择2012年6月~2013年12月在我科年龄相关性白内障138例178眼分为两组进行超声乳化联合人工晶体植入术,透明角膜切口1.8mm组(81眼)与3.0mm组(97眼),比较术后1w、1个月和3个月角膜散光及泪液分泌变化。结果术后1w 1.8mm组与3.0mm组之间比较角膜散光及泪液分泌变化有显著性差异(<0.01);术后1个月组间比较有差异(0.05)。结论切口大小对白内障术后角膜散光及泪液分泌变化的影响不同。其中1.8mm微切口角膜散光及泪液分泌变化的影响相对小,而3.0mm透明角膜切口影响大,3个月后这种差异逐渐消除。“,”Objective The purpose of this study was to investigate the ef ect of endocapsular phacoemulsification cataract extraction and intraocular lens (IOL) implantation with a 1.8mm or 3.0mm clear corneal incision on corneal astigmatism and schirmer.Methods From June 2012 to December 2013, 138 age-related patients (178 eyes) were randomly distributed into two groups. 1.8mm-group comprised 81 eyes that had a silicone IOL inserted through a 1.8mm sutureless clear corneal incision, while, 3.0mm-group comprised 97 eyes through a 3.0mm clear corneal incision. Postoperatively, we determined the changes in the corneal astigmatism and schirmer. at 1 wk, 1 and 3 mons respectively.Results In both groups, postoperatively at 1week, There were statistical y significant dif erences ( <0.05) in the corneal astigmatism and schirmer. while, there were statistical y minimal dif erences ( 0.05)between two groups at 3 months.Conclusion This study confirms that incision size has strong impact on the corneal higher-order aber ations, especial y, 3.0mm incision caused significant dif erences in the corneal astigmatism and schirmer. compared with 1.8mm micro-incision, therefore, micro-incision is very beneficial for clinical use in phacoemulsification. March and later gradual y eliminate such dif erences.
