目的探讨听觉脑干诱发电位(ABR)在言语障碍儿童中的变化规律及应用价值。方法对2006年1月-2007年12月来本院神经电生理室就诊的言语障碍儿童35例进行ABR分析,采用MEB-9100型诱发电位仪,观察ABR的阈值、波形、潜伏期(PL)、波间期(IPL)。结果35例言语障碍儿童ABR正常9例,异常26例。分化状况好18耳次,分化尚可10耳次,分化差26耳次,无分化16耳次。ABR分级正常18耳次,轻度异常11耳次,中度异常24耳次,重度异常17耳次。ABR异常26例Ⅰ波双耳PL延长11例,单耳PL延长7例;Ⅲ波双耳PL被动延长11例,单耳PL被动延长7例;Ⅴ波双耳PL被动延长11例,单耳PL被动延长7例。Ⅰ~ⅢIPL双耳延长3例,Ⅲ~ⅤIPL双耳延长1例,单耳延长2例,Ⅰ~ⅤIPL双耳延长1例,单耳延长1例。结论ABR对言语障碍儿童听力损害及神经传导经路异常者提供有价值的客观指标,且特别适用于婴幼儿和难于受测试者。
Objective To investigate the changes of auditory brainstem response potential (ABR) in speech-impaired children and its clinical value. Methods A retrospective analysis was performed on 35 children with speech impairment who visited our hospital from January 2006 to December 2007 in our hospital. The MEB-9100 evoked potentials were used to observe the ABR threshold, waveform, latency (PL) Wave interval (IPL). Results 35 cases of speech impairment children ABR normal in 9 cases, abnormal 26 cases. Differentiation is good 18 ears, differentiation can still 10 ears, poor differentiation 26 ears, no differentiation 16 ears. ABR normal 18 ears, mild abnormalities 11 ears, moderate abnormalities 24 ears, severe abnormalities 17 ears. ABR abnormalities in 26 cases Ⅰ wave binaural PL extended in 11 cases, monophasic PL extended in 7 cases; Ⅲ wave binaural PL passive extended in 11 cases, monophasic PL passive extended in 7 cases; Ⅴ wave binaural PL passive extended in 11 cases, monaural PL passive extension of 7 cases. Three cases of Ⅰ ~ ⅢIPL binaural prolongation, one case of Ⅲ ~ ⅤIPL binaural prolongation, two cases of monaural prolongation, one case of Ⅰ ~ ⅤIPL double ear prolongation and one case prolongation of single ear. Conclusion ABR provides valuable objective indicators for hearing impaired children with speech impairment and abnormal nerve conduction pathways, and is especially suitable for infants and young children and difficult subjects.
