目的探讨CTA评价主动脉瓣钙化对主动脉瓣功能的影响。方法搜集464例行CTA检查的患者资料,分为主动脉瓣膜钙化组61例和非钙化组403例。测量所有患者的收缩期末主动脉瓣开放角及主动脉瓣开口面积并测量瓣膜的厚度;记录主动脉瓣膜钙化的位置、钙化CT值及钙化面积。用Agataston方法定量分析钙化并计算钙化积分,分析钙化积分与各参数的关系。结果钙化瓣膜开放角小于非钙化瓣膜开放角,钙化瓣膜开放角与钙化位置有关。主动脉瓣钙化以后瓣及瓣膜根部多见。钙化瓣膜的厚度大于非钙化瓣膜的厚度,钙化组瓣膜开口面积小于非钙化的瓣膜开口面积。主动脉瓣各位置的钙化积分与主动脉瓣开放角、钙化积分与瓣膜开口面积及瓣膜厚度具有良好的关联性,且回归关系具有统计学意义。结论主动脉瓣钙化限制主动脉瓣的运动,以主动脉根部钙化为著。主动脉瓣钙化积分越高可能表明主动脉瓣开放残余角越大,主动脉瓣狭窄越重。主动脉瓣开放角的测量可能对瓣膜置换术有指导意义。
Objective To investigate the effect of CTA on the function of aortic valve after aortic valve calcification. Methods A total of 464 patients undergoing CTA were enrolled and divided into aortic valve calcification group (n = 61) and non-calcification group (n = 403). All patients were measured at the end of systolic aortic valve opening angle and aortic valve opening area and measure the thickness of the valve; record aortic valve calcification, calcification calcification value and calcification area. The calcifications were quantified using the Agataston method and the calcification scores were calculated. The relationship between calcification scores and parameters was analyzed. Results The open angle of calcified valve was smaller than that of non-calcified valve, and the open angle of calcified valve was related to the calcified position. Aortic valve calcification and valvular root more common. The thickness of the calcified valve is greater than that of the non-calcified valve, and the calcified valve has a smaller opening area than the non-calcified valve opening. There was a good correlation between calcification scores and aortic valve open angle, calcification score and valve opening area and valve thickness at all locations of aortic valve, and the regression relationship was statistically significant. Conclusion Aortic valve calcification limits the aortic valve movement, aortic root calcification. The higher aortic valve calcification score may indicate that the greater the aortic valve residual remnant angle, the more severe the aortic stenosis. Valuation of the aortic valve open angle may be instructive for valve replacement.
