目的:探讨儿童肾积水对尿路感染的影响,总结临床治疗方法。方法:选择2014年6月-2016年9月间于我院就诊的96例肾积水患儿为研究对象,回顾分析他们的尿培养、尿常规和泌尿系统彩超等临床资料,统计分析临床治疗结果。结果:重度肾积水患儿尿路感染的阳性率为20.0%,显著高于轻度肾积水患儿尿路感染阳性率2.8%,二组差异具有统计学意义(P<0.05);合并有输尿管扩张肾积水患儿尿路感染率为18.5%,显著高于无输尿管扩张患儿尿路感染率5.6%,二组差异具有统计学意义(P<0.05);出现尿路感染患儿中有7例培养出细菌,且同同期本院普通尿路感染细菌谱比较差异不具有统计学意义(P>0.05)。讨论:儿童肾积水病情越重以及合并有输尿管扩张病症患儿出现尿路感染的风险更高,且主要以大肠埃希菌等革兰阴性菌为主要病原菌,同其他普通尿路感染病病原菌谱相比,差异不具有统计学意义。因此,在临床治疗儿童肾积水的时候,需要结合患儿肾积水实际情况来做好尿路感染的防治工作。
Objective: To investigate the effect of hydronephrosis in children on urinary tract infection and to summarize the clinical treatment. Methods: Ninety-six children with hydronephrosis who were treated in our hospital from June 2014 to September 2016 were selected as the study objects. Clinical data of urinary culture, urinary routine and urological color Doppler ultrasound were retrospectively analyzed. The clinical data of clinical treatment result. Results: The positive rate of urinary tract infection in children with severe hydronephrosis was 20.0%, which was significantly higher than that of mild hydronephrosis (2.8%). The difference between the two groups was statistically significant (P <0.05) The urinary tract infection rate in children with ureteral dilatation and hydronephrosis was 18.5%, which was significantly higher than that in children without ureteral dilatation (5.6%). The difference between the two groups was statistically significant (P <0.05) In 7 cases, bacteria were cultured, and there was no significant difference in bacterial spectrum between ordinary urinary tract infection and normal hospital in the same period (P> 0.05). Discussion: Children with hydronephrosis more severe and children with ureteral dilatation disease associated with urinary tract infection risk is higher, and mainly Escherichia coli and other Gram-negative bacteria as the main pathogen, with other common urinary tract infection pathogens The differences were not statistically significant. Therefore, in the clinical treatment of children with hydronephrosis, the need to combine the actual situation of children with hydronephrosis do a good job in prevention and treatment of urinary tract infection.
