目的 :探讨超声探头在不同程度的加压扫查状态下 ,对乳腺肿瘤彩色血流信号分级及脉冲多普勒参数造成影响的程度 ,使超声工作者对不当的操作手法造成的乳腺肿瘤血流动力学影响引起高度重视 ,避免方法学上的人为因素导致操作者对肿瘤性质的判断出现错误 ,以提高超声诊断乳腺肿瘤的准确性。方法 :选取 30例在非加压状态下彩色多普勒显示为 级及 级 (Adler标准 )血流信号的乳腺肿瘤进行不同程度的加压扫查 ,观察肿瘤彩色血流信号分级及脉冲多普勒参数 (VS、 VD、 RI、 VS/VD)的变化。结果 :在 级、 级压力状态下 ,乳腺肿瘤血流分级无明显改变。 级以上压力状态下 ,彩色血流信号分级有不同程度的减低。在 级压力状态下 ,脉冲多普勒参数多普勒血流各参数无显著性差异 (P>0 .0 5 )。从 级压力开始 ,多普勒血流参数与 0级状态下比较均有显著性差异 (P<0 .0 0 1) ,VS、VD与压力等级呈负相关 ,随压力增大 ,数值逐级变小。但二者并不成比例下降。随压力增大 ,VS/VD逐级增大 ,显示 VD较 VS下降速度更快。RI与压力等级呈正相关 ,压力越大 ,RI值逐级变大。结论 :加压扫查对乳腺肿瘤的供血状态有明显影响 ,压力越高、影响越大。对乳腺肿瘤探察时应对探头压力进行控制 ,防止人为因素对肿瘤血供产生影响

Conclusions: Pressure scan has a significant effect on the status of breast tumor blood supply, the higher the pressure, the greater the impact. Detection of breast tumors should probe pressure control to prevent human factors on the impact of tumor blood supply
