目的探讨双手微小切口白内障超声乳化联合小梁切除术在青光眼白内障联合手术中的疗效。方法回顾性分析我院2008年09月～2010年06月行双手微小切口白内障超声乳化联合小梁切除术的30例患者的临床资料,观察手术后眼压、视力变化和并发症情况。结果 30眼中0.02～0.1者3眼(10.00%);0.1～0.3者8眼(26.67%);0.3～0.5者12眼(40%);0.5～0.8者5眼(16.67%),其中>1.0者2眼(6.66%);术后平均眼压稳定在(13.57±3.31)mmHg;术后出现角膜水肿2眼,虹膜反应和晶状体前渗出膜6眼,后囊膜混浊3眼,经积极治疗后明显改善。结论微小切口白内障超声乳化联合青光眼小梁切除手术对急性闭角型青光眼疗效较为确切,虽然也存在一定的并发症,但及时处理并发症,仍可获得满意疗效,是一种理想的手术方式。
Objective To investigate the effect of micromanipulation combined with trabeculectomy in glaucoma and cataract surgery. Methods A retrospective analysis of our hospital from September 2008 to June 2010 in both hands micro incision cataract phacoemulsification combined with trabeculectomy in 30 patients with clinical data were observed after surgery, intraocular pressure, visual acuity and complications. Results Among the 30 eyes, there were 3 eyes (10.00%) in 0.02 ~ 0.1 eyes, 8 eyes (26.67%) in 0.1 ~ 0.3 eyes, 12 eyes (40%) in 0.3 ~ 0.5 eyes and 5 eyes (16.67%) in 0.5 ~ 0.8 eyes, (2) eyes (6.66%). The mean intraocular pressure was stable at (13.57 ± 3.31) mmHg after operation. Corneal edema was observed in 2 eyes, iris reaction and lenticular exudate in 6 eyes and posterior capsular opacity in 3 eyes. After treatment significantly improved. Conclusion The treatment of acute angle-closure glaucoma with minimal incision phacoemulsification combined with glaucoma trabeculectomy is more exact. Although there are some complications, it is still an ideal surgical method to treat the complications in time.
