目的为了解基层医生全科医学在职培训效果,为今后的进一步培训提供经验和决策依据。方法采用问卷法对1996~1998年间参与首都医科大学全科医师培训中心系统培训项目的北京学员进行了培训效果调查。结果绝大部分学员的知识、技能、临床思维、交流能力与工作态度等都有所改善;从事医疗与护理工作的学员,其各项能力改善均较明显;具有中专和大学学历者,培训后能力改善程度较高;超过1/3的学员近期在诊治病人次数、建立家庭保健合同户数、日常处理病种、慢性病管理人数和服务对象的满意度等全科医疗服务相关指标上有所增加,特别是服务满意和慢性病管理人数增加较多。结论全科医学以人与健康为中心的观念和相关知识技能的培训,在基层在职培训中收到了良好效果;培训应以临床能力为基础,逐步扩大到“一体化”服务能力;对不同类型的社区卫生服务人员培训应区别对待。
The purpose is to understand the effectiveness of on-the-job training for general practitioners in primary medicine and provide experience and decision-making basis for further training in the future. Methods Questionnaires were used to investigate the training effectiveness of Beijing trainees who participated in the training program for general practitioner training center of Capital Medical University from 1996 to 1998. Results Most of the trainees’ knowledge, skills, clinical thinking, communication skills, and work attitudes were improved; those who were engaged in medical and nursing work were all significantly improved in their abilities; those with technical secondary school and university qualifications were trained. The degree of improvement in post-competency is high; more than one third of the students have recently been related to general medical service indicators such as the number of patient visits, the number of home health care contract establishments, the number of daily treatment items, the number of chronic disease managers, and the satisfaction of clients. Increases, especially service satisfaction and chronic disease management, increase more. Conclusion The concept of general human medicine and the training of related knowledge and skills centered on people’s health have received good results in on-the-job training at the grass-roots level; training should be based on clinical capabilities and gradually expanded to “integrated” service capabilities; The training of community health service personnel should be treated differently.
