目的分析急性脑梗死的经颅多普勒超声(TCD)与CT诊断结果,比较两者之间的差异。方法选择荆州市第一人民医院2008年8月-2012年7月收治的急性脑梗死患者160例,均经颅脑CT检查确诊,按照梗死面积分为三组。采用经颅多普勒超声诊断仪进行TCD检查,观察各组患者TCD检查结果的异常率。结果急性脑梗死患者TCD异常率为71.88%(115/160)。其中大面积梗死患者TCD异常率为91.38%(53/58);小面积脑梗死患者TCD异常率为61.82%(34/55);腔隙性脑梗死患者TCD检查率为59.57%(28/47),其中大面积梗死患者TCD异常率明显较高,差异有统计学意义(P<0.05)。结论 TCD检查可直接获得患者脑动脉血流动力参数,对急性脑梗死尤其是大面积梗死不失为是一种重要的辅助诊断方法。
Objective To analyze the results of transcranial Doppler ultrasound (TCD) and CT in acute cerebral infarction and to compare the differences between them. Methods The first People’s Hospital of Jingzhou City, August 2008 - July 2012 admitted to patients with acute cerebral infarction in 160 cases, were confirmed by brain CT examination, divided into three groups according to the infarct size. Transcranial Doppler ultrasound diagnostic TCD examination was performed to observe the abnormal rate of TCD examination results in each group of patients. Results The abnormal rate of TCD in patients with acute cerebral infarction was 71.88% (115/160). TCD abnormality rate was 91.38% (53/58) in large area infarction patients, 61.82% (34/55) in small area cerebral infarction patients, and 59.57% (28/47) in lacunar infarction patients ). Among them, the abnormal rate of TCD in patients with large infarction was significantly higher (P <0.05). Conclusion TCD can directly obtain cerebral arterial hemodynamic parameters in patients with acute cerebral infarction, especially after a large infarct is an important diagnostic method.
