目的探讨下肢静脉顺行造影(AV)、彩色多普勒超声(CDUS)及经皮氧分压(Tc PO2)对明确下肢静脉性溃疡静脉病变的临床应用价值。方法通过分析AV与CDUS两种检查结果 ,对53例共54条下肢静脉性溃疡患肢静脉病变的解剖资料进行分析,另对10例溃疡周边皮肤及对侧同一截面正常皮肤行Tc PO2测定,并对测定结果进行分析。结果 AV与CDUS两种检查方法对明确下肢静脉性溃疡静脉病变差异无统计学意义(P>0.05),Kappa=0.658,有较高一致性。溃疡周边皮肤与对侧正常肢体同一截面皮肤经Tc PO2测定差异有统计学意义(P<0.05)。结论 AV与CDUS均可作为下肢静脉性溃疡静脉病变的可靠检查方法 ,两者协同诊断率高。Tc PO2是检测局部微循环状态的有力手段,可能作为预测静脉性溃疡发生、发展及预后情况的可靠指标。
Objective To investigate the clinical value of paraventricular vein angiography (AV), color Doppler ultrasonography (CDUS) and transcutaneous oxygen pressure (Tc PO2) in the diagnosis of venous lesions of lower extremity venous ulcers. Methods According to the results of AV and CDUS, the anatomical data of 53 cases with venous leg ulceration in 54 cases were analyzed. Tc PO2 was also measured in 10 cases of peripheral skin of the ulcer and normal skin on the contralateral side. And the results of the analysis. Results There was no significant difference between AV and CDUS in detecting the venous lesions of lower extremity venous ulcer (P> 0.05). Kappa = 0.658, with a high consistency. The difference of Tc PO2 in the skin with the same cross-section between the skin around the ulcer and the normal contralateral side was statistically significant (P <0.05). Conclusions Both AV and CDUS can be used as a reliable method for the detection of venous lesions of venous leg ulcers. The co-diagnosis rate of both is high. Tc PO2 is a powerful means to detect the state of local microcirculation, which may be used as a reliable indicator to predict the occurrence, development and prognosis of venous ulcer.
