目的检测溃疡性结肠炎患者(ulcerative colitis,UC)血清中IL-33的表达,探讨其与UC炎症活动期的关系及在激素治疗前后和临床转归的变化。方法应用酶联免疫吸附法(ELISA)检测88例UC患者血清IL-33水平,其中活动期65例,缓解期23例;其中40例中重度UC患者接受泼尼松治疗,30例健康人为正常对照组。病例组分别根据临床分期、活动期病变程度进行分组,比较各组血清中IL-33表达的差异。结果病例组活动期血清IL-33为[(59.54±19.37)ng/L,显著高于缓解期[(22.26±5.23)ng/L]及对照组[(20.84±3.25)ng/L],差异均有统计学意义(t=10.840,P<0.001;t=9.082,P<0.01);活动期,轻、中、重度UC患者血清IL-33水平差异有统计学意义(F=98.620,P<0.01);激素敏感型UC患者经泼尼松治疗后血清IL-33水平较治疗前下降,差异有统计学意义(t=8.040,P<0.01);激素抵抗或依赖型UC患者经泼尼松治疗前后血清IL-33水平差异无统计学意义(t=0.163,P>0.05)。UC活动期血清IL-33水平与UC活动性的改良Mayo评分呈正相关(r=0.762,P<0.01)。结论 IL-33的表达与UC的发生、发展呈正相关,且与UC的炎症活动明显相关,可作为判断UC患者病变严重程度和激素治疗敏感性的指标之一。
Objective To detect the expression of IL-33 in the serum of patients with ulcerative colitis (UC) and to investigate the relationship between IL-33 and the inflammatory activity of UC and the clinical outcome before and after hormone therapy. Methods Serum levels of IL-33 in 88 patients with UC were detected by enzyme-linked immunosorbent assay (ELISA), of which 65 were active and 23 were remission. Among 40 patients with moderate-to-severe UC, prednisone and 30 healthy controls were normal Control group. The cases were divided into groups according to the clinical stage and the degree of active lesion. The differences of IL-33 expression in serum of each group were compared. Results The serum level of IL-33 in active cases was (59.54 ± 19.37) ng / L, significantly higher than that in remission (22.26 ± 5.23) ng / L and [(20.84 ± 3.25) ng / L) (T = 10.840, P <0.001; t = 9.082, P <0.01). The levels of IL-33 in active, mild, moderate and severe UC patients were significantly different (F = 98.620, P < 0.01). The level of serum IL-33 in hormone-sensitive UC patients after prednisone treatment was significantly lower than that before treatment (t = 8.040, P <0.01). In patients with steroid-responsive UC, prednisone There was no significant difference in serum IL-33 level before and after treatment (t = 0.163, P> 0.05). The level of IL-33 in UC patients was positively correlated with the modified Mayo score of UC activity (r = 0.762, P <0.01). Conclusions The expression of IL-33 is positively correlated with the occurrence and development of UC, and is significantly correlated with the inflammatory activity of UC. It may be used as one of the indicators to judge the severity of the disease and the sensitivity of hormone therapy in UC patients.
