目的 探讨C-反应蛋白(C-reactive protein ,CRP)、可溶性髓样细胞触发受体-1(soluble myeloid cell trigger receptor-1 ,sTREM-1)、肺表面活性蛋白A(pulmonary surfactant protein A,SPA)和白介素-6(interleukin-6 ,IL-6)在早产儿急性呼吸窘迫综合征(acute respiratory distress syndrome ,ARDS)中的早期变化及意义.方法 选取内蒙古医科大学附属医院2016年1月—2018年1月ARDS患儿110例,根据病情严重程度诊断标准将其分为轻度组(n=36)、中度组(n=42)及重度组(n=32).选择50例同期健康儿童为对照组.于患儿入院当天分别检测患者血清中CRP、sTREM-1、SPA和IL-6的含量进行统计分析.结果 与对照组相比,随着ARDS疾病程度的加重,患儿血清CRP、sTREM-1、SPA和IL-6含量显著升高(P<0.05).患儿血清CRP、sTREM-1、SPA和IL-6联合检测的灵敏度明显优于各指标的单独检测,CRP、sTREM-1、SPA和IL-6单独检测的诊断特异性明显低于各组的联合检测(P<0.05).ARDS患儿血清CRP、sTREM-1、SPA和IL-6含量与患儿的疾病程度存在显著的正性相关关系,P<0.05.结论 ARDS患儿血清中CRP、sTREM-1、SPA和IL-6含量呈异常升高,且随着早产儿AR?DS疾病程度的加重而升高,对于辅助诊断早产儿ARDS具有一定的临床意义,值得推广研究.“,”objective To investigate the early changes and effects significance C-reactive protein (CRP), soluble my?eloid cell-triggered receptor-1 (sTREM-1), pulmonary surfactant protein A (SPA) and interleukin-6 (IL-6) in preterm infants with acute respiratory distress syndrome. Methods From January 2016 to January 2018, 110 children with AR?DS were divided into three groups: mild group (n=36), moderate group (n=42) and severe group (n=32). Fifty healthy children in the same period were selected as the control group. On the day of admission, The levels of CRP,sTREM-1, SPA and IL-6 in serum of the patients were analyzed statistically. Results Compared with the control group, the levels of serum CRP,sTREM-1,SPA and IL-6 increased significantly with the aggravation of ARDS disease (P<0.05). The di?agnosissensitivity of combined detection of CRP,sTREM-1,SPA and IL-6 was significantly better than that of each in?dex, and the diagnosisspecificity of CRP,sTREM-1,SPA and IL-6 was significantly lower than that of each group (P<0.05). There was a significant positive correlation between the levels of serum CRP,sTREM-1,SPA and IL-6 and the degree of disease (P<0.05). Conclusion The levels of serum CRP,sTREM-1,SPA and IL-6 in children with ARDS are abnormally elevated, which increases with the aggravation of ARDS disease in preterm infants. It is of clinical signifi?cance for the assistant diagnosis of ARDS in premature infants, and it is worthy to be popularized and studied.
