万古霉素血药浓度监测与临床用药行为相关问题研究

目的探讨万古霉素血药浓度的监测结果以及临床用药的相关问题分析。方法对110例应用万古霉素并监测血药浓度的住院患者进行调查分析,统计患者的治疗效果以及万古霉素血药浓度的监测结果。结果治疗后,总有效79例(71.82%),其中显效52例(47.27%)、有效27例(24.55%)、无效31例(28.18%)。万古霉素血药浓度峰值<25 mg/L的患者60例(54.55%),谷值<5 mg/L的患者56例(50.91%)。结论由于万古霉素的治疗指数较窄,且患者的个体差异等因素会直接影响到万古霉素的治疗效果,患者出现不良反应的几率相对较高。因此,医护人员要根据万古霉素的监测结果以及患者的自身症状调整给药方案,尽量避免患者出现不良反应,促进其早日康复。 Objective To investigate the monitoring results of vancomycin blood concentration and the related problems of clinical medication. Methods A total of 110 hospitalized patients with vancomycin and blood plasma concentration monitoring were surveyed and analyzed. The therapeutic effect of vancomycin and the monitoring results of vancomycin plasma concentration were statistically analyzed. Results After treatment, the total effective rate was 79 (71.82%), of which 52 (47.27%) were effective, 27 (24.55%) were effective and 31 (28.18%) were ineffective. Sixty patients (54.55%) with vancomycin peak plasma concentration <25 mg / L, and 56 patients (50.91%) with trough <5 mg / L vancomycin. Conclusion Due to the narrower therapeutic index of vancomycin and the individual differences among patients, the treatment effect of vancomycin will be directly affected. The incidence of adverse reactions in patients is relatively high. Therefore, health care workers should be based on vancomycin monitoring results and the patient’s own symptoms to adjust the dosing regimen, try to avoid adverse reactions in patients and promote their early recovery.
