目的了解急性白血病不同类(亚)型及不同病期患者肿瘤坏死因子(TNF)在mRNA水平表达的差异。方法应用RT-PCR半定量法检测急性白血病患者65例(初治49例,复发8例,缓解8例)外周血或骨髓血单个核细胞TNFmRNA表达量,并以15例献血员及健康志愿者为对照。结果(1)急性白血病初治组的TNFmRNA表达阳性率虽低于对照组(P<0.01),但平均表达量显著高于对照组(P<0.01),其中M5a的TNFmRNA表达量显著高于M5b及其他各类(亚)型(P<0.01)。(2)急性白血病缓解期TNFmRNA表达有下降的趋势,复发时回升,并发感染时明显增高。结论TNFmRNA的检测在临床上有助于某些急性白血病的分型,在理论上提示TNF可能在某些类型急性白血病的发生、发展上起重要作用
Objective To understand the difference of tumor necrosis factor (TNF) mRNA expression in patients with different types of subtypes and different stages of acute leukemia. Methods Semiquantitative RT-PCR method was used to detect the expression of TNFmRNA in peripheral blood or bone marrow mononuclear cells in 65 patients with acute leukemia (n = 49 cases, relapse 8 cases, remission 8 cases), and 15 blood donors and healthy volunteers. For comparison. Results (1) The positive rate of TNFmRNA expression in the initial group of acute leukemia was lower than that in the control group (P<0.01), but the average expression level was significantly higher than that in the control group (P<0.01). The expression of TNFmRNA in M5a was significant. Significantly higher than M5b and other types (subtypes) (P<0.01). (2) The expression of TNFmRNA in acute leukemia has a decreasing trend during remission, and it rises at the time of relapse. Conclusion The detection of TNFmRNA is clinically helpful for the classification of some acute leukemias. Theoretically, it suggests that TNF may play an important role in the occurrence and development of certain types of acute leukemia.
