目的探讨肝细胞癌(HCC)患者血清中Glypican-3(GPC3)蛋白的表达和意义,及其与AFP联合检测在HCC中的诊断价值。方法收集35例HCC患者,24例肝良性病变患者以及6例正常对照术前血清标本,采用western blotting检测血清中GPC3蛋白,化学发光法检测血清中AFP。结果35例HCC患者、24例肝良性病变患者以及6例正常对照GPC3蛋白的阳性率分别为51%、0%、0%,AFP的阳性率分别为60%、38%、0%。单独用GPC3蛋白或AFP对HCC诊断的灵敏度分别为51%、60%(P>0.05),特异度分别为100%、70%(P<0.05),Youden指数分别为0.51、0.30(P<0.05);而用GPC3蛋白+AFP联合检测诊断HCC的灵敏度、特异度和Youden指数分别为91%、70%和0.61。结论GPC3蛋白在HCC患者血清中有较高的表达,可作为HCC早期诊断的标志物,GPC3蛋白+AFP联合检测可提高HCC的确诊率。
Objective To investigate the expression and significance of Glypican-3 (GPC3) in the serum of patients with hepatocellular carcinoma (HCC) and its diagnostic value in combined detection of AFP and HCC. Methods Serum samples of 35 patients with HCC, 24 patients with benign liver lesions and 6 normal controls were collected. GPC3 protein was detected by western blotting and AFP was detected by chemiluminescence. Results The positive rates of GPC3 protein in 35 patients with HCC, 24 patients with benign liver disease and 6 normal controls were 51%, 0% and 0%, respectively. The positive rates of AFP were 60%, 38% and 0%, respectively. The sensitivity of diagnosing HCC with GPC3 protein alone or AFP alone was 51%, 60% (P> 0.05), specificity was 100%, 70% (P <0.05), Youden index was 0.51,0.30 ). The sensitivity, specificity and Youden index of combined diagnosis of HCC with GPC3 + AFP were 91%, 70% and 0.61, respectively. Conclusion GPC3 protein is highly expressed in the serum of HCC patients and can be used as a marker for early diagnosis of HCC. Combined detection of GPC3 protein and AFP can increase the diagnosis rate of HCC.
