目的探讨高频震荡通气(HFOV)治疗对新生儿急性呼吸窘迫综合征(ARDS)呼吸道分泌物白细胞介素-6(IL-6)的影响。方法将54例新生儿ARDS按照治疗方法不同分为2组,在一般治疗的基础上,对照组21例采用传统机械通气治疗,治疗组33例采用HFOV治疗,比较两组患儿呼吸道分泌物IL-6水平,记录上机后24、48、72 h PA-aO2及上机时间、用氧时间、住院时间。结果治疗组存活患儿治疗后呼吸道分泌物IL-6水平较对照组显著降低,同时PA-aO2明显降低,上机时间、用氧时间和住院时间均较对照组缩短,与对照组相比,其差异具有统计学意义(P<0.05)。结论 HFOV可显著改善呼吸道分泌物IL-6水平,缓解新生儿ARDS患儿的肺氧合功能,缩短治疗时间。
Objective To investigate the effects of high frequency oscillatory ventilation (HFOV) on respiratory secretions of neonatal acute respiratory distress syndrome (ARDS), interleukin-6 (IL-6). Methods 54 neonates with ARDS were divided into two groups according to different treatment methods. On the basis of general treatment, 21 cases in the control group were treated by traditional mechanical ventilation, and 33 cases in the treatment group were treated by HFOV. The respiratory secretions IL -6 level, recorded after the machine 24,48,72 h PA-aO2 and the machine time, oxygen time, hospital stay. Results The levels of IL-6 in respiratory secretions of survivors survived in the treatment group were significantly lower than those in the control group, PA-aO2 was significantly decreased, and the uptime, oxygen time and hospitalization time were shorter than those in the control group. The difference was statistically significant (P <0.05). Conclusions HFOV can significantly improve the level of IL-6 in respiratory secretions, relieve the pulmonary oxygenation function and shorten the treatment time in neonates with ARDS.
