目的分析双侧肺同期手术中不同体位及单肺通气对患者呼吸力学的影响。方法选取射阳县中医院2014年3月—2016年12月收治的双侧肺同期手术患者70例,通过气道旁路使用旁气流通气,持续记录比较患者不同体位、不同肺通气PIP、Pplat、Raw。结果双肺通气VCV1,患者平卧位变侧卧位后,PIP、Pplat、Raw上升(P<0.05);变换通气方法后,VCV2较VCV1的PIP、Pplat、Raw下降(P<0.05)。单肺通气后,PIP、Pplat、Raw较双肺通气上升(P<0.05);变换通气方法后,PCV较VCV2 PIP、Pplat、Raw下降(P<0.05)。结论双侧肺同期手术中不同体位及单肺通气均可对患者呼吸力学产生有利影响。
Objective To analyze the influence of different position and single lung ventilation on respiratory mechanics in patients with bilateral lung surgery. Methods Seventy patients with bilateral lung surgery admitted from Sheyang County Hospital of Traditional Chinese Medicine from March 2014 to December 2016 were enrolled in this study. The paraventricular airway ventilation was used to record the changes of PIP, Pplat Raw. Results After ventilated with VCV1 in both lungs, the PIP, Pplat and Raw levels increased in the supine position (P <0.05). After the ventilation method was changed, VCV2 decreased compared with VCV1 (P <0.05). Compared with VCV2 PIP, Pplat, Raw (P <0.05), ventilation of PIP, Pplat and Raw increased after ventilation (P <0.05). Conclusion Different positions and single lung ventilation during bilateral simultaneous lung surgery may have a beneficial effect on the respiratory mechanics of patients.
