目的观察海岛模拟湿热环境下脊髓火器伤后脊髓组织的病理形态学变化,分析其损伤机制与原因。方法选用南方实验用猪构建湿热环境下脊髓火器伤模型,于6 h后取脊髓组织观察其大体形态变化及病理形态学改变。结果脊髓连续性遭到严重破坏,发生碎裂、坏死、缺损,电镜下可见脊髓硬脊膜血管急剧扩张,硬膜下广泛瘀血,灰质中神经元细胞变性等严重病理改变。结论直接致伤作用、瞬时空腔效应、压力波致伤效应和血液扰动效应等四种机制使脊髓受到严重损伤,而湿热环境下这种损伤更为严重。
Objective To observe the morphological changes of spinal cord after spinal cord firearm wounds simulated by hot and humid island in the island, and to analyze its mechanism and causes. Methods Southern experimental pigs were used to construct the model of firearm injury in spinal cord in damp-heat environment. The spinal cord tissues were taken at 6 hours to observe the general morphological changes and pathomorphological changes. Results The continuity of the spinal cord was severely damaged, with the occurrence of fragmentation, necrosis and defect. The severe dilatation of the spinal dural blood vessels, extensive blood stasis under the epidural membrane and degeneration of neuronal cells in gray matter were observed under electron microscope. Conclusions The four mechanisms of direct injury, instantaneous cavity effect, pressure wave injury and blood disturbance effect severely damage the spinal cord, while the injury is more severe in the hot and humid environment.
