目的比较单采血小板与手工分离血小板的质量及输注疗效,为临床合理应用两种血小板制品提供参考依据。方法随机抽查作者医院2015-01/2016-12月入库的100袋单采血小板和100袋400ml全血手工分离的浓缩血小板的血小板、红细胞和白细胞含量,比较两种血小板的质量。随机选取54例输注单采血小板患者和52例输注手工分离血小板患者,分别计算输注24h后的血小板校正增加指数(corrected count increment,CCI)和血小板回收率(percentage of platelet recovery,PPR),并统计输血不良反应的发生例数,判断其输注疗效。结果 1U单采血小板与12U手工分离血小板的血小板含量差异无统计学意义(P>0.05);单采血小板的白细胞含量、红细胞含量低于手工分离血小板,差异有统计学意义(P<0.05)。两组输注血小板患者的输注疗效、输血反应发生率的差异均无统计学意义(P>0.05)。血液系统疾病患者与非血液系统疾病患者输注血小板的疗效差异无统计学意义(P>0.05)。结论手工分离血小板具有一定的有效性和安全性,可作为单采血小板紧缺状况下有力的补充,帮助缓解当今血小板供应紧张的局面。
Objective To compare the quality of single-platelet and manual-separated platelets and the effect of infusion, and provide reference for clinical application of two kinds of platelet products. Methods The contents of platelets, erythrocytes and leucocytes in 100 platelets collected from the hospital from January 2015 to December 2016 - December and 100 platelets from 400 ml whole blood were compared randomly to compare the quality of the two platelets. Fifty-four patients with single platelet transfusion and 52 patients with manual platelet transfusion were selected randomly and the corrected CCI and percentage of platelet recovery (PPR) , And statistics of the number of adverse reactions occurred transfusion, to determine the infusion efficacy. Results The platelet content of 1U apheresis platelets and 12 U manual detached platelets showed no significant difference (P> 0.05). The leukocyte content and the erythrocyte content of apheresis platelets were lower than those of manual platelet deparaffinization (P <0.05). There was no significant difference in the incidence of transfusion and transfusion between the two groups of patients receiving transfusion of platelets (P> 0.05). There was no significant difference in the efficacy of transfusion of platelets between patients with hematologic diseases and those with non-hematological diseases (P> 0.05). Conclusion The manual isolation of platelets has some effectiveness and safety, which can be used as a potent supplement to apheresis platelets to help alleviate the current shortage of platelet supply.
