目的:探讨次极量平板运动试验在评价飞行学员心电图T波变化的价值。方法:选择心电图Ⅱ、Ⅲ、aVF导联T波低平飞行学员33例,分别进行登梯运动试验和次极量平板运动试验,之后继续完成近2年的基础体能训练。比较次极量平板运动试验、登梯运动试验与基础体能训练后心电图检查T波合格情况,以及次极量平板运动试验与登梯运动试验对Ⅱ、Ⅲ、aVF导联T波低平的评价结果。结果:完成近2年基础体能训练后T波合格率,与次极量平板运动试验后4min、6min,以及与登梯运动试验后2min、4min、6min比较,差异均非常显著(P<0.01);而与次极量平板运动试验后2min比较,则差异不显著(P>0.05)。在评价Ⅱ、Ⅲ、aVF导联T波低平方面,次极量平板运动试验后2min的敏感度最高,漏诊率最低;而登梯运动试验后不同时段的敏感度均较低,且漏诊率较高。结论:次极量平板运动试验后2min与基础体能训练后,心电图检查T波合格情况接近,因此,在招飞体检时建议采用次极量平板运动试验评价心功能。
Objective: To investigate the value of submaximal treadmill exercise test in assessing T wave changes of electrocardiogram in flight students. Methods: 33 electrocardiogram Ⅱ, Ⅲ, aVF lead T-wave and low-level flight trainees were selected. They were used to carry out the ladder exercise test and the submaximal treadmill exercise test, respectively, and then completed the basic physical training for the past 2 years. Comparative submaximal treadmill exercise test, ECG T-wave after passing the boarding case basis physical training and exercise test, and submaximal treadmill exercise test and stair climbing exercise test in Ⅱ, Ⅲ, aVF lead T wave flat Evaluation result. Results: the T-wave pass rate, and submaximal treadmill exercise test after the completion of almost 2 years of basic physical training 4min, 6min, and a 2min, 4min, 6min, the difference was significant after stair climbing exercise test (P <0.01) ; While 2min after submaximal treadmill exercise test, the difference was not significant (P> 0.05). In evaluating the low-level T wave of Ⅱ, Ⅲ and aVF leads, the sensitivity of 2min after submaximal treadmill exercise test was the highest, and the misdiagnosis rate was the lowest. However, the sensitivity of different stages after the stair treadmill exercise test was lower, and the misdiagnosis rate Higher. CONCLUSION: T wave passed by electrocardiogram nearly 2min after submaximal treadmill exercise test. Therefore, it is recommended to use submaximal treadmill exercise test to evaluate cardiac function during the physical examination.
