Objective To study the expression of intercellular adhesion molecule-1 (ICAM 1) and IL-1α in the microvasculature of polymyositis and explore the role of vascular factors in polymyositis. Methods Ten cases of polymyositis and six noninflammatory myopathies were collected. The area and density of CD34, ICAM 1 and IL 1α positive microvessel were calculated by image analysis, and the quantitative analysis and comparison were made. Results Compared with noninflammatory myopathies, the microvessel density in the muscle of patients with polymyositis was significantly decreased (P = 0 009). The expression of ICAM 1 and IL 1α in the microvascular endothelium was stronger than that in the control group. Conclusion The myocytic dysfunction in patients with polymyositis is impaired, suggesting that vascular factors may be involved in the pathogenesis of polymyositis.
