目的 探讨天津地区汉族Graves病 (GD)白细胞减少易感性与HLA DRB1基因多态性的关联。方法 采用聚合酶链反应 序列特异性引物 (PCR SSP)方法检测 45例GD白细胞减少患者、5 0例GD白细胞正常患者和 90名正常对照的HLA DRB1等位基因频率。结果 (1)在不考虑白细胞变化的情况下 ,GD患者DRB1 0 8基因频率明显高于对照组 (P <0 .0 1,RR =2 .6 2 ) ,DRB1 0 7基因频率明显低于对照组 (P <0 .0 1,RR =0 .2 4)。 (2 )GD白细胞减少组DRB1 0 8(P <0 .0 1,RR =4.17)和DRB1 15 (P <0 .0 5 ,RR =1.6 9)基因频率较对照组显著增高 ,DRB1 0 7基因频率明显低于对照组 (P <0 .0 1,RR =0 .13)。 (3)GD白细胞减少组DRB1 0 8基因频率 (P <0 .0 1)和DRB1 15基因频率 (P <0 .0 5 )均明显高于白细胞正常组 ,DRB1 0 9(P <0 .0 5 )基因频率明显低于白细胞正常组。结论 天津地区汉族GD白细胞减少易感性与HLA DRB1 0 8,HLA DRB1 15基因频率增加有关 ;GD的保护性与HLA DRB1 0 7基因频率减少有关
Objective To investigate the association between leukopenia susceptibility and HLA DRB1 gene polymorphism in graves disease (GD) of Han nationality in Tianjin area. Methods The frequencies of HLA DRB1 alleles in 45 GD patients, 50 GD patients and 90 normal controls were detected by polymerase chain reaction sequence specific primers (PCR SSP). Results (1) The frequency of DRB1 0 8 gene in GD patients was significantly higher than that in control group (P <0.01, RR = 2.62), and the frequency of DRB1 0 7 gene was significantly lower than that of control Group (P <0. 01, RR = 0.24). (2) The frequency of DRB1 0 8 (P <0.01, RR = 4.17) and DRB1 15 (P <0.05, RR = 1.6 9) in GD leukopenia group was significantly higher than that in control group The frequency was significantly lower than the control group (P <0.01, RR = 0.13). (3) The frequency of DRB1 0 8 gene (P <0.01) and the frequency of DRB1 15 gene in GD leukopenia group were significantly higher than those in normal leukocyte group, DRB1 0 9 (P <0. 0) 5) Gene frequency was significantly lower than normal leukocyte group. Conclusions The susceptibility to GD leukopenia in Han population in Tianjin is related to the increased frequency of HLA DRB1 0 8 and HLA DRB1 15 genes; the protective effect of GD is related to the decrease of HLA DRB1 0 7 gene frequency
