目的评价有创与无创序贯性机械通气在慢性阻塞性肺疾病(COPD)合并急性呼吸衰竭患者救治中的疗效。方法比较24例采用有创与无创序贯性机械通气治疗患者(序贯组)和22例采用单纯机械通气治疗患者(常规组)住院时间、有创通气时间、总机械通气时间、呼吸机相关性肺炎(VAP)发生率、再插管率的差异。结果有创与无创序贯性机械通气治疗COPD合并急性呼吸衰竭疗效显著。序贯组总机械通气时间[(11±4)d]、住院时间[(17±3)d]、有创通气时间[(5.0±2.2)d]较传统组[分别为(17±8)d、(27±6)d、(13.5±5.7)d]均缩短(P<0.05),VAP发生率低于传统组(P<0.05)。结论有创与无创序贯性机械通气治疗COPD合并急性呼吸衰竭具有很好的临床实用价值。
Objective To evaluate the efficacy of invasive and noninvasive sequential mechanical ventilation in the treatment of patients with chronic obstructive pulmonary disease (COPD) complicated with acute respiratory failure. Methods Twenty-four patients (sequential group) with invasive and non-invasive sequential ventilation and 22 patients with conventional mechanical ventilation (conventional group) were compared in terms of length of hospital stay, invasive ventilation, total mechanical ventilation, Vocal pneumonia (VAP) incidence, re-intubation rate differences. Results The results of invasive and noninvasive sequential mechanical ventilation in the treatment of COPD with acute respiratory failure were significant. The mean duration of mechanical ventilation ([11 ± 4] d], hospital stay [(17 ± 3) d], duration of invasive ventilation [(5.0 ± 2.2) d] d, (27 ± 6) d, (13.5 ± 5.7) d] were all shortened (P <0.05). The incidence of VAP was lower than that of the traditional group (P <0.05). Conclusion Both invasive and noninvasive sequential mechanical ventilation have good clinical value in the treatment of COPD with acute respiratory failure.
