目的探讨经会阴超声测量宫颈长度对预测延期妊娠引产的价值。方法对120例需引产的延期妊娠初产妇,于引产前2h内经会阴超声测量宫颈长度,同时由产科医生专人进行Bishop评分。随访引产结果。结果78例引产成功,宫颈平均长度为(23±9)mm;42例引产失败,宫颈平均长度为(32±5)mm,引产成功组宫颈长度明显短于引产失败组,两组比较有显著性差异。以宫颈长度≤30mm为界限值,预测引产成功的灵敏度为0.89,特异度0.85,阳性预测值0.92,阴性预测值0.81。结论对于初产妇而言,经会阴超声测量宫颈长度可以评价宫颈成熟度,可以预测延期妊娠的引产效果,优于Bishop评分。
Objective To investigate the value of measuring the length of the cervix by transperineal ultrasound in predicting delayed labor induced labor. Methods A total of 120 primiparae of late pregnancy who needed induction of labor were enrolled. The length of cervix was measured by perineal ultrasound within 2 hours before induction of labor. Conclusions For primiparous women, cervical maturation can be assessed by measuring the length of the cervix by perineal ultrasound and predicting induction of labor during delayed pregnancy, outperforming the Bishop score.
