目的研究Gesell发育量表在婴幼儿脑瘫治疗前后评估中的效果及价值。方法选取2015年1月-2016年7月该院诊治的婴幼儿脑瘫患儿100例,从运动、语言、认知及情感等方面进行康复治疗及早期干预,采用Gesell发育量表评估患儿治疗前后智力水平及智力结构,采用粗大运动功能评估等对患儿运动功能进行分级。结果偏瘫型患儿大运动、应物、言语及应人评分显著高于痉挛型双瘫型、不随意运动型及共济失调型(P<0.05);痉挛型双瘫型患儿细动作评分显著高于偏瘫型、不随意运动型及共济失调型(P<0.05);脑瘫患儿治疗后大运动、细动作、应物、言语及应人评分均显著低于对照组(均P<0.05);脑瘫患儿治疗后发育月龄水平(DA)显著低于治疗前(P<0.05);患儿治疗后发育商水平(DQ)显著高于治疗前(P<0.05);中文版脑瘫粗大运动功能分级系统(GMFCS)中患儿等级越高,DA及DQ水平越低(P<0.05)。结论不同类型脑瘫患儿功能障碍分布存在差异,患儿经系统治疗后采用Gesell发育量表评估能了解其预后情况。
Objective To study the effect and value of Gesell Scale in the evaluation of infant cerebral palsy before and after treatment. Methods From January 2015 to July 2016, 100 children with cerebral palsy who were diagnosed and treated in the hospital from January 2015 to July 2016 were treated with rehabilitation and early intervention in terms of exercise, language, cognition and emotion, and the Gesell development scale was used to evaluate the treatment of children Before and after the level of intelligence and mental structure, the use of gross motor function assessment of children with motor function rating. Results The children with hemiplegia had greater scores of exercise, speech, speech and coping than those with spastic bipolar disorder and involuntary exercise and ataxia (P <0.05). The children with spastic diplopia (P <0.05); children with cerebral palsy had significantly lower scores of major exercise, fine motor, response, speech and their response after treatment than those of the control group (all P < (P <0.05). The developmental age (DA) of children with cerebral palsy after treatment was significantly lower than that before treatment (P <0.05). The level of developmental quotient (DQ) in children with cerebral palsy after treatment was significantly higher than that before treatment The higher the level of children in the GMFCS, the lower the DA and DQ levels (P <0.05). Conclusion There are differences in the distribution of dysfunction in children with different types of cerebral palsy. The children can be evaluated by Gesell scale after system therapy to understand their prognosis.
