目的 观察丙酮酸对急性阻塞性肠缺血再灌注 (I/R)的影响。方法 5 0只Wistar大鼠(幼龄 40只和成年 10只 )分为 5组 :手术对照组、缺血组、再灌注组、丙酮酸组、成年动物再灌注组。在 6 0min缺血和 12 0min再灌注后 ,进行生化和组织病理学检测。结果 缺血组血清谷丙转氨酶(ALT)值和肠粘膜损害较手术对照组明显升高 (P <0 .0 5 ) ,超氧化物岐化酶 (SOD)活性无明显变化 ;再灌注组血清ALT值明显升高 ,SOD活性明显减低 ,肠粘膜评分为 3.95± 0 .6 9,与手术对照组和缺血组相比较差异均有显著性 (P <0 .0 1) ;丙酮酸组ALT值、SOD活性和肠粘膜评分与再灌注组相比均有明显改善 (P <0 .0 1) ;幼龄再灌注组ALT值和肠粘膜评分均明显高于成年动物再灌注组 (P <0 .0 5 )。结论 幼鼠肠缺血可以引起肠粘膜和肝脏功能损伤 ,再灌注则加重这种损伤并导致肠、肝、肺组织病理形态学改变 ,其机制与再灌注时氧自由基大量产生有关。丙酮酸可以减轻再灌注损伤 ;在相同条件下 ,幼龄动物比成年动物对肠I/R更为敏感。
Objective To observe the effect of pyruvate on acute obstructive intestinal ischemia-reperfusion (I / R). Methods Fifty Wistar rats (40 in juvenile and 10 in adult) were divided into 5 groups: operation control group, ischemia group, reperfusion group, pyruvate group and adult animal reperfusion group. Biochemical and histopathological examinations were performed at 60 min ischemia and 120 min reperfusion. Results Serum alanine aminotransferase (ALT) and intestinal mucosal lesion were significantly increased in ischemic group compared with the control group (P <0.05), and the activity of superoxide dismutase (SOD) had no significant change. Serum ALT value was significantly increased, SOD activity was significantly reduced, intestinal mucosa score was 3.95 ± 0.69, compared with the surgical control group and ischemic group were significantly different (P <0.01); pyruvate group ALT SOD activity and intestinal mucosa score were significantly improved compared with reperfusion group (P <0.01). ALT value and intestinal mucosa score of young reperfusion group were significantly higher than those of reperfusion group (P < 0 .0 5). Conclusions Gut ischemia can cause intestinal mucosal and hepatic injury in reperfusion rats. Reperfusion can aggravate this injury and cause histopathological changes in intestine, liver and lung. The mechanism is related to the massive production of oxygen free radicals during reperfusion. Pyruvate can reduce the reperfusion injury. Under the same conditions, young animals are more sensitive to intestinal I / R than adult animals.
