目的比较多层螺旋CT、磁共振成像(MRI)及经直肠超声(ERUS)评价直肠癌术前T分期的准确性。方法回顾性分析2012年4月至2016年4月收治的62例直肠癌患者的临床资料,均行多层螺旋CT、MRI及ERUS检查,总结直肠癌患者三种检查的影像表现,根据手术病理学结果,比较三种检查方法的T分期符合率。统计学检验水准取α=0.05,进行分割χ~2检验时,校正为α’=0.0125。结果 62例患者的手术病理结果显示,T_1期14例,T_2期12例,T_3期36例。以病理结果为准,多层螺旋CT诊断直肠癌的T分期符合率为72.6%(45/62);MRI的T分期符合率为88.7%(55/62);ERUS的T分期符合率为91.9%(57/62)。多层螺旋CT的术前T分期诊断符合率低于MRI及ERUS(P均<0.0125),MRI及ERUS的T分期诊断符合率相当(P>0.0125)。结论 MRI及ERUS诊断直肠癌术前T分期的准确性明显高于多层螺旋CT,且二者效果相当,各有优势与局限性,为提高直肠癌患者的术前T分期准确性以制定最佳治疗方案,可将二者结合实现互补,获取更全面信息。
Objective To compare the accuracy of preoperative T staging of rectal cancer with multislice spiral CT, magnetic resonance imaging (MRI) and transrectal ultrasound (ERUS). Methods The clinical data of 62 patients with rectal cancer who were treated from April 2012 to April 2016 were retrospectively analyzed. All patients underwent multi-slice spiral CT, MRI and ERUS. The imaging findings of the three examinations were summarized. Rational results, comparing the three methods of T staging coincidence rate. Statistics test level to take α = 0.05, split χ ~ 2 test, corrected to α ’= 0.0125. Results The pathological results of 62 patients showed that there were 14 cases in T_1, 12 cases in T_2 and 36 cases in T_3. According to the results of pathology, the coincidence rate of multislice spiral CT in the diagnosis of rectal cancer was 72.6% (45/62), the coincidence rate of T staging in MRI was 88.7% (55/62), and the coincidence rate of T stage in ERUS was 91.9 % (57/62). The coincidence rate of preoperative T stage diagnosis of MSCT was lower than that of MRI and ERUS (all P <0.0125). The coincidence rate of MRI and ERUS was the same (P> 0.0125). Conclusion The accuracy of MRI and ERUS in diagnosing rectal cancer preoperative T staging is significantly higher than that of multislice spiral CT. Both of them have the same effect and have their own advantages and limitations. In order to improve the preoperative T staging accuracy of rectal cancer, Good treatment options, the two can complement each other for more comprehensive information.
