新生儿窒息常合并心脑等多系统损害,本文通过检测血清CK-MB(心型)和心电图,评价心肌损害情况。36例窒息新生儿,轻度窒息15例,重度窒息21例,分别于治疗前、治疗7天后测CK-MB、心电图,治疗前轻度组CK-MB水平为31.5±15.4、异常率46.7%,重度组分别为50.5±20.3、85.7%,两者有显著性差异(P<0.05),治疗后CK-MB水平及异常率明显下降(P<005),治疗前心电图仅5例异常,治疗后完全恢复。随诊1~4月无CK-MB及心电图异常,结果提示,特异性心肌酶(CK-MB)是反映窒息新生儿心肌损害的一个敏感指标,心电图可反映心肌损害,但敏感性较低。观察和随诊说明心肌损害恢复较快,预后良好。
Neonatal asphyxia often combined with heart and brain damage and other systems, this paper by detecting serum CK-MB (cardiogram) and electrocardiogram, evaluation of myocardial damage. 36 cases of asphyxia, 15 cases of mild asphyxia and 21 cases of severe asphyxia. CK-MB and ECG were measured before treatment and 7 days after treatment respectively. The level of CK-MB in mild group before treatment was 31.5 ± 15.4, The abnormal rate was 46.7% in the severe group and 50.5 ± 20.3 and 85.7% in the severe group (P <0.05), and the CK-MB level and abnormal rate were significantly decreased after treatment P <0 05), before treatment ECG abnormalities in only 5 cases, completely recovered after treatment. There was no CK-MB and ECG abnormalities in follow-up from January to April. The results suggested that CK-MB was a sensitive indicator of myocardial damage in neonates with asphyxia. ECG could reflect myocardial damage, but the sensitivity was low. Observation and follow-up showed that myocardial damage recovered faster and the prognosis was good.
