目的通过程序设计,实现基于WEB的最佳住院日和药占比的计算机辅助决策。方法从深圳某区区域卫生信息平台数据仓库中提取四家医院的病案首页数据和HIS数据,以ICD编码、年份、医院、病区、医生、病种等多个维度,计算住院日和药占比二个指标的中位数和算术平均数,按中位数绘制总收入线和成本线,计算指标的中位数和算术平均数的最小值。结果总收入线和成本线的交点就是效益临界点;总收入线在成本线以下的病种适合在门诊治疗;总收入线在成本线以上的病种效益较好;最佳住院日和合适的药占比分别是指标的中位数和算术平均数的最小值。结论最佳住院日和药占比的计算机辅助决策,能科学地指导医院及科室合理缩短平均住院日,降低药占比,优化医疗资源配置。
Objective To realize the computer-aided decision-making of the best hospitalization days and medicine proportion based on WEB by program design. Methods The first case data and HIS data of the four hospitals were extracted from the data warehouse of a regional health information platform in a certain area of Shenzhen. The hospitalization days and drug use were calculated based on the dimensions of ICD, year, hospital, ward, doctor and disease. The median and arithmetical mean of the two indicators are plotted against the median by the total revenue and cost lines and the median and the arithmetic mean of the indicators are calculated. Results The total income line and the intersection of the cost line is the critical point of efficiency; the total income line below the cost line is suitable for outpatient treatment; the total income line is better than the cost line; the best hospital stay and the appropriate Medications are the median of the indicators and the arithmetic mean, respectively. Conclusion Computer-assisted decision-making of the best hospitalization days and the proportion of medicine can guide hospitals and departments scientifically to shorten the average length of stay, reduce the proportion of medicine and optimize the allocation of medical resources.
