目的探讨尿液中性粒细胞明胶酶相关载脂蛋白(u NGAL)检测对急性肾损伤(AKI)早期诊断价值。方法选取2014年8月至2015年6月入住ICU后7 d内符合AKI诊断标准的80例患者作为研究组,选取同期未发生AKI的80例患者作为对照组。检测两组入住ICU即刻(30 min)内、第24、48、72 h的u NGAL、尿素氮(BUN)、血肌酐(Cr)水平,并进行比较。结果与入住ICU即刻相比,第24、48、72 h研究组u NGAL水平均明显上升,且随时间增长而逐渐上升,差异有统计学意义(P<0.05);两组BUN、Cr水平在第24 h均未见明显变化,差异未见统计学意义(P>0.05);研究组u NGAL水平在第24、48、72 h较对照组均明显上升,BUN、Cr水平在各时段较对照组均明显上升,差异有统计学意义(P<0.05)。结论 u NGAL作为AKI的早期标志物比传统的BUN、Cr更具有临床诊断价值。
Objective To investigate the value of urinary neutrophil gelatinase-associated apolipoprotein (u NGAL) in the early diagnosis of acute kidney injury (AKI). Methods Eighty patients with AKI within 7 days after admission to ICU from August 2014 to June 2015 were selected as the study group and 80 patients without AKI during the same period were selected as the control group. The levels of u NGAL, BUN and Cr at the 24th, 48th and 72th hour after ICU admission were detected within 30 minutes after ICU admission. Results The level of u NGAL in the 24th, 48th and 72th hour groups increased significantly compared with that in the ICU immediately after admission, and gradually increased with time increasing (P <0.05). The levels of BUN and Cr (P> 0.05). The level of u NGAL in study group was significantly higher than that in control group at 24,48,72 h, and the level of BUN and Cr in each group was higher than that in control group Group were significantly increased, the difference was statistically significant (P <0.05). Conclusion u NGAL as an early marker of AKI than the traditional BUN, Cr more clinical diagnostic value.
