目的探讨压力调节容量控制(PRVC)通气模式在新生儿呼吸窘迫综合症(NRDS)中的应用及临床效果。方法 20例新生儿呼吸窘迫综合症(NRDS)患儿采用PRVC通气呼吸支持,监测患儿上机前后动脉血PaO2、PaCO2、SaO2、pH等变化,并观察临床治疗效果及呼吸机相关并发症。结果患儿上机6 h后PaO2、SaO2、pH上升,PaCO2下降(P<0.05),机械通气时间为95 h(中位数),临床治愈率为85%,其中12例发生呼吸机相关性肺炎,4例动脉导管开放,2例颅内出血,2例慢性肺疾病,无患儿发生气漏。结论 PRVC模式治疗能够有效改善NRDS的肺部呼吸功能,呼吸机相关并发症发生率较低,具有较好临床效果。
Objective To investigate the application and clinical effect of pressure regulative volume control (PRVC) ventilation in neonatal respiratory distress syndrome (NRDS). Methods 20 cases of neonatal respiratory distress syndrome (NRDS) children were treated with PRVC ventilation and respiratory support. PaO2, PaCO2, SaO2, pH and other changes of arterial blood were monitored before and after treatment. The clinical effects and ventilator-related complications were observed. Results The PaO2, SaO2, pH increased, PaCO2 decreased (P <0.05), the mechanical ventilation time was 95 h (median), the clinical cure rate was 85%, of which 12 patients had ventilator-related Pneumonia, 4 cases of patent ductus arteriosus, 2 cases of intracranial hemorrhage, 2 cases of chronic lung disease, and no air leak in children. Conclusions PRVC model treatment can effectively improve the lung respiratory function of NRDS, and the incidence of ventilator-related complications is low, which has a good clinical effect.
