目的探讨糖尿病合并冠心病不同NYHA心功能分级者尿微量蛋白的水平。方法选取2010年9月~2011年9月于某院进行治疗的354例糖尿病合并冠心病患者为观察组,同期的354例单纯冠心病患者为对照组,将两组人员不同心功能分级及总计的尿β2-微球蛋白、α1-微球蛋白及微量白蛋白的含量及阳性率进行检测及比较,并将两组的不良心脑血管事件发生率进行比较。结果观察组的不同心功能分级及总计的尿β2-微球蛋白、α1-微球蛋白及微量白蛋白的含量及阳性率均高于对照组,且其不良心脑血管事件发生率也明显高于对照组,另外观察组中NYHA心功能分级Ⅳ级上述指标均高于Ⅱ~Ⅲ级患者,P均﹤0.05,差异有统计学意义。结论糖尿病合并冠心病患者心功能分级较高者的尿微量蛋白水平高于心功能分级较低者,且明显高于单纯冠心病患者。
Objective To investigate the levels of urinary microalbuminuria in patients with diabetes and coronary heart disease under NYHA class. Methods 354 patients with diabetes mellitus and coronary heart disease treated in a hospital from September 2010 to September 2011 were selected as the observation group. 354 patients with simple coronary heart disease in the same period were selected as the control group. The differences in cardiac function scores and total Urinary β2-microglobulin, α1-microglobulin and microalbumin content and positive rate were detected and compared, and the adverse cardiac and cardiovascular events in both groups were compared. Results In the observation group, the scores of different cardiac functions and the total urinary β2-microglobulin, α1-microglobulin and microalbumin contents were higher in the observation group than those in the control group, and the incidence of adverse cardiovascular and cerebrovascular events was also significantly higher In the control group, NYHA cardiac function grade Ⅳ in the other observation groups were higher than those in grade Ⅱ ~ Ⅲ, P <0.05, the difference was statistically significant. Conclusion The urinary microalbuminuria in patients with diabetes mellitus with coronary heart disease higher than that in patients with lower cardiac function was significantly higher than those with simple coronary heart disease.
