目的 应用射血力 (F)评价肺动脉高压时右心室收缩功能。方法 通过对 2 0 0 0年 10月至 2 0 0 1年 12月在我院住院的 10 5例左向右分流型先天性心脏病 (CHD)进行彩色多谱勒超声心动图测定计算射血力 (F)和心导管检查 ,探讨肺动脉高压 (PH)时F的特点 ,并与血液动力学参数对照 ,评价F与dp/dtmax相关性及F在不同PH时的特点。采用方差分析病例组及对照组的差异及以线性相关分析F与dp/dtmax的关系 ,P <0 .0 5为有统计学意义。结果 病历组与对照组相比 ,病例组的F明显增加 (P <0 .0 0 1)。病例组亚组之间比较 ,无PH组F低于中、重PH组 (P <0 .0 5 )。重PH组F较中PH组F低 (P <0 .0 5 )。与dp/dtmax呈正相关 (r=0 .85 ,P <0 .0 5 )。结论 F与dp/dtmax相关性好 ,可以较好地反应右心室的收缩功能。PH组F明显增高 ,严重肺高压时F仍较对照组高 ,但较轻、中PH组低
Objective To evaluate right ventricular systolic function in pulmonary hypertension by using ejection force (F). Methods Color Doppler echocardiography was performed on 105 cases of left to right shunt congenital heart disease (CHD) hospitalized in our hospital from October 2000 to December 2001 to calculate the ejection (F) and cardiac catheterization to explore the characteristics of F in pulmonary hypertension (PH), and compared with hemodynamic parameters to evaluate the relationship between F and dp / dtmax and the characteristics of F in different PH. The variance analysis of cases and control groups and the difference between the linear correlation analysis of F and dp / dtmax, P <0. 05 for statistical significance. Results Compared with the control group, the case group had significantly higher F (P <0.01). There was no significant difference between group PH and group PH (P <0. 05). The heavy PH group F was lower than the middle PH group F (P <0.05). And dp / dtmax was positively correlated (r = 0.85, P <0.05). Conclusion F and dp / dtmax correlation is good, can better reflect the right ventricular systolic function. PH group F was significantly higher, severe pulmonary hypertension F still higher than the control group, but lighter, medium PH group low
