目的探讨慢性肾病患者抗栓药物治疗方案。方法临床药师参与1例慢性肾病合并冠心病和心房颤动患者的抗栓治疗,对阿司匹林、氯吡格雷、低分子肝素以及华法林的药物选择和剂量调整进行了评估。结果和结论患者病情好转出院,且住院期间未出现出血症状。临床药师协助医师在积极抗栓同时,在最大程度上保证了用药安全。
Objective To investigate the antithrombotic therapy in patients with chronic kidney disease. Methods Clinical pharmacists participated in antithrombotic therapy in one patient with chronic kidney disease complicated with coronary heart disease and atrial fibrillation. The drug selection and dose adjustment of aspirin, clopidogrel, low molecular weight heparin and warfarin were evaluated. RESULTS AND CONCLUSIONS Patients were well discharged and discharged without symptoms of bleeding during hospitalization. Clinical pharmacists to assist physicians in the active antithrombotic at the same time, to ensure maximum medication safety.
