目的 探讨 16层螺旋CT同时扫描肺动脉和下肢静脉的联合成像技术。方法 4 0例疑为肺动脉栓塞 (简称肺栓塞 )的病人于 16层螺旋CT上行肺动脉和下肢静脉联合成像。CT后处理包括最大密度投影 (MIP)、多平面重建 (MPR)及容积再现 (VR)。采用t检验进行统计学分析。结果肺栓塞、下肢静脉血栓同时存在者 2 5例 ,单纯下肢静脉血栓者 8例 ,单纯肺栓塞者 2例 ,二者均正常者 5例。不同重建层厚均可清晰显示肺动脉栓子。肺动脉、下肢静脉增强后的CT值显著高于栓子的CT值。MPR、MIP、VR三者对肺栓塞的显示率分别为 10 0 %、10 0 %、6 5 % ,下肢静脉血栓的显示率分别为 10 0 %、6 0 %、5 0 %。结论 16层螺旋CT联合肺动脉、下肢静脉成像将为肺栓塞的诊断提供新的方法。
Objective To investigate the combined imaging technique of simultaneous 16-slice spiral CT scanning of pulmonary artery and venous leg. Methods Twenty-four patients with suspected pulmonary embolism (pulmonary embolism) were enrolled in this study. CT post-processing includes maximum density projection (MIP), multiplanar reconstruction (MPR) and volume rendering (VR). T test was used for statistical analysis. Results There were 25 cases of pulmonary embolism and venous thrombosis in the lower extremities, 8 cases of simple venous thrombosis in the lower extremities, 2 cases of simple pulmonary embolism, and 5 cases both of them were normal. Different reconstruction thickness can clearly show pulmonary emboli. Pulmonary artery, lower extremity venous enhanced CT value was significantly higher than the CT value of the emboli. The rates of pulmonary embolism in MPR, MIP and VR were 100%, 100% and 65% respectively, and the rates of venous thrombosis in lower extremity were 100%, 60% and 50% respectively. Conclusion 16-slice CT combined with pulmonary artery and venous vein imaging will provide a new method for the diagnosis of pulmonary embolism.
